
Service Plans are the objectives and goals written by the Residential Counselor from observations and assessments of a resident’s first 30 days and comprehensive evaluation. These objectives and goals are designed individually for each resident to successfully complete the Meadowlark program. There are a total of 8 areas covered by the service plans. Though they are designed for the need of each client individually, there are some universal objectives that each resident will follow. Below are the 8 areas on which the service plans are based and examples of some universal long-term goals that each resident is expected to achieve.

| Drugs and Alcohol | Learn how to cope without use of drugs and alcohol. Submit to random urinalysis a minimum of one time per month. |
| Education | Demonstrate the ability to attend public school regularly and achieve passing grades in all of her classes. Earn a minimum of 9 high school credits during her placement. |
| Family / Aftercare | Demonstrate the ability to communicate and visit with her family or aftercare resource while following all laws and Meadowlark rules/ expectations. Participate in family groups and gain communication and problem-solving skills. |
| Legal | Follow all laws while at Meadowlark and in community. Demonstrate the ability to make progress on her treatment goals and resolve problems without acting out or running away. |
| Medical / Psychiatric | Adopt a healthy lifestyle Participate in an active exercise program and develop positive, long-term fitness and eating habits. |
| Abuse / Relationships | Learn how to develop healthy relationships with positive peers and family members. Demonstrate the ability to address relationship/abuse issues while in placement. |
| Social Skills / Behavioral | Demonstrate the ability to accept responsibility for her actions, without minimizing or justifying her involvement Learn how to stop her irresponsible thinking and replace it with responsible thinking. |
| Gender / Cultural | Develop a strong sense of her own female identity through participation in groups and activities which promote greater understanding of self-expression and her own unique qualities. Develop an understanding of her own culture and increase her awareness of others’ culture. |
